We support and encourage our residents to be as independent as they can and to live as healthy and enjoyable a life as possible.

Care at Provenance is delivered by our own staff including access to full multi-disciplinary team under the clinical leadership of our Medical Director. All our staff goes through the same initial training so the complete team: clinicians and support staff, are able to interact and support the residents, providing a holistic and personal service. 

We know that no two illnesses are alike, nor do they create the same challenges for the individual therefore our services are dynamic, progressive and most of all person-centred.

We pride ourselves on delivering the highest standard of care at all times.  We offer bespoke individualised care programmes provided by a dedicated, trained team of highly skilled staff. Our staff works closely with residents to plan and supervise day-to-day care.

One third of people with a diagnosis of dementia live in residential care.  The vast majority of these are classified in the early or mid-stages, therefore the clinical services for dementia are generally aimed at the early and mid-stages of Alzheimer’s. With the progression of long term degenerative neurological conditions each stage brings increased challenges both clinically and behaviorally, increasing levels of dependency and need for specialist interventions, which often results in exclusion from mainstream services.

Our services include five care pathways each with a distinct care model:

  • High Dependency 1 – male only service with high therapeutic needs and challenging behaviours
  • High Dependency 2 – female only service for those with high therapeutic needs and challenging behaviours
  • Nursing – patients with high physical nursing needs as a result of degenerative neurological conditions
  • Residential rehabilitation – for those who were previously detained under the MHA,(Mental Health Act) 1983 who are now on a pathway to independent living also for those with neurological degenerative diseases
  • Low dependency – a step down to more independent living and a pathway back to the community
