WHY PROVENANCE

We provide specialised care to residents requiring complex care for difficult and diverse behaviours as a result of a variety of diagnosis.

Most of our residents come to us when neither the provisions of traditional care homes and mental illness units are suitable for their specific care. We welcome residents who require more complex care where higher staffing levels are appropriate. We embrace those who are difficult to place, treatment resistive, or those with degenerative conditions for whom current provision is sparse.

Our resident population have moderate to severe communication difficulties, physical health issues, cognitive problems and behavioural problems.  Our care models will address all of the above with the ultimate aim of providing specialist care whilst maintaining a level of independence they can achieve at each stage of the illness.  From assessment to discharge, our care models provide for the changing nature of the resident’s presentation.

Our highly skilled team work alongside our residents and their family, the NHS/ funding authority and specialists to develop and continually review personalised care plans based on the individual’s changing needs.

We help residents to build on their own strengths so that they can prepare for a life with their condition. Our residents come to us confident that they will be comprehensively assessed and cared for with personalised care plans compiled with our expert team.
